Commitment Policy

FC West Soccer Club Commitment Policy

Congratulations on being selected as a member to play on one of FC West Soccer Club's Competitive teams.

Playing on one of these teams is not a right, but rather a privilege conditioned on compliance at all times with FC West Code of Conduct, Financial, Academic Standards, and Concussion \Trauma Policies.

1. As a Player you promise and agree to comply fully with the FC West Code of Conduct which expressly prohibits possession and/or use of alcohol, tobacco, and controlled substance (unless prescribed specifically for Player by a licensed physician).

2. Player and Parent(s) agree that FC West may terminate a Player's privilege to play with FC West and forfeit all Club fees and Team fees paid upon violation of FC West's Code of Conduct.

3. Player and Parent(s) agree to comply fully with the rules, requirements, and regulations of the individual leagues the teams are playing in, along with those of the USSF.

4. Players are not allowed to be a guest player for any other club without the prior written consent of your team's coaching staff and the Club's head coach or DOC.

5. Player and Parent(s) acknowledge and recognize that soccer is a sports activity with inherent risks with a possibility of physical injury, and hereby release, discharge, and otherwise indemnify FC West, their sponsors, affiliated organizations, board members, officers, employees, coaches, managers, volunteers and associated personnel of FC West, against any claim, liability for personal injury, death or property damage by or on behalf of the player or Parent as a result participation in a FC West program.

6. Player and Parent(s) acknowledge FC West's Concussion \Trauma Policy.

7. Player and Parent(s) consent to have an athletic trainer, coach, team manager, emergency medical technician, nurse, medical treatment facility/hospital, and/or doctor of medicine or dentistry or associated personnel to provide emergency transportation, emergency care, medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. Player and Parent(s) agree to provide proof of insurance and will provide a further consent to medical treatment in a form required by FC West Soccer & US Club Soccer and/or USSF.

8. Player and Parent(s) acknowledge that the Club and related persons, including its board members, officers, employees, coaches, managers, volunteers, and associated personnel of FC West are not responsible for loss or theft of personal property.

9. Player and Parent(s) agree to comply with FC West's Financial Policy which requires all player account to be in good standing. Good standing means that all fees (registration, uniforms, tournament, team fees, etc.) are paid in full or a payment installment plan that is satisfactory to FC West is in place and current.

Player and Parent(s) agree that FC West may suspend a player's playing privileges during any period that their account is not in good standing and could result in termination of a Player's ability to play with FC West. There are no refunds upon termination of an account.

10. In the event of a player leaving FC West their account is required to be paid in full before their player card will be released.

11. Player agrees and promises to uphold the Academic standards and guidelines of the school they attend. Parent(s) agree to promptly notify FC West if it becomes necessary and in the best interest of the player to suspend Player's playing privileges during any period when Player is not in compliance.

12. Player agrees and promises to uphold the Standards of Student Conduct of the school they attend. Parent(s) agree to promptly notify FC West if Player is not in compliance with their school's Standards of Student Conduct. Both Player and Parent(s) agree that FC West may suspend Player's playing privileges during any period when Player is not in compliance.
